From our custom-designed programming to our hand-picked staff, we serve more as your partner than your provider when it comes to creating meaningful days, productive years, and a fulfilling life for your loved one living with intellectual and developmental disabilities. 

We are licensed by the Indiana Family and Social Services Administration and offer a full spectrum of programming through the Community Integration and Habilitation Waiver, and the Family Supports Waiver.

We are accredited by the Indiana Family and Social Services Association (FSSA), the US Department of Labor, and the The Council on Quality and Leadership (CQL).

Our Mission: To empower individuals with disabilities, their families, and those living with mental illness to achieve personal success by providing choices and opportunities.

Our Passion: Our directors, staff, volunteers, and funding partners are all passionate about helping those we serve create meaningful days, productive years, and, ultimately, more fulfilling lives.

Our People: We support both children and adults whose lives are impacted by mild to profound disabilities. Our service area reaches those living in and around Northwest Indiana.

Residential Habilitation and Support (RHS): Provides up to 24-hours of support and services which are designed to ensure the health, safety, and welfare of the consumer. We help consumers acquire, improve, and retain skills necessary to lead successful lives in their homes. 

Day Habilitation-Individual (DHI): Provides a variety of assistance outside of the consumer’s home to support learning and provide assistance in the areas of self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. The activities are intended to build relationships and natural supports within the consumer’s community.

Respite (RSPO): Provides temporary relief to the unpaid caregiver(s) allowing them to take a break while knowing that their loved one is provided with care. We offer these supports within the community or within the consumer’s or family’s home.

Participant  Assistance and Care (PAC): Provides support for activities of daily living, self-care, and mobility with the hands-on assistance, prompting, reminders, supervision, and monitoring needed to ensure the health, safety, and welfare of the consumer.  

Workplace Assistance: Provides personal care services and/or supports to assist a consumer to accomplish tasks that they would be able to complete if they did not have a disability. 

 Extended Services: Provides work related supports to assist a consumer to be as independent as possible while maintaining competitive employment that is integrated in the community. 

Recreational Therapy: Provides opportunities to improve a consumer’s functioning and independence and reduce or eliminate the effects of a consumer’s disability.
