Your Partner In Care
Home Based Support Services (HBS) provides self-directed assistance (SDA), support planning and supports to adults with developmental disabilities to prevent or delay out of home residential services. Individuals and families choose from an array of services that include needs assessment for services, day programs, direct services provided by domestic or other employees, as well as a variety of therapy and other services, such as adaptive equipment. Home Based Support Services are tailored to meet individuals’ evolving needs and preferences for support, while maintaining their current residences.

We will help you assess your budget, review your options, and provide recommendations regarding what services will meet your family’s needs. Program Highlights
- Serving individuals with Adult Home-Based Support (AHBS) Medicaid Waivers
- Services can be provided in the home of the individual or in the community
- Services to promote skill development and community linkage including the State appointed fiscal agent ACES$
- Coordinate with other programs in which individual/family is already involved
- Provide skills training to help individuals and families to recruit and hire Personal Support Workers (PSW) as well as provide information on effective communication
- Assist individuals and family with prior approval requests, such as adaptive equipment, home & vehicle modifications, assistive technology, and therapies

We offer practical skills training to enable individuals and families to independently direct and manage Medicaid Waiver services.
