What to expect at your intake appointment
An intake specialist will meet with you for 1-2 hours to discuss your reasons for seeking services. Be prepared to talk about your past and your current concerns. The intake specialist will review with you all of our program options and explore what might be the best fit for you. After your appointment your intake specialist will keep in contact with you until you are placed in our services.
For this appointment, you are required to bring the following:
- Identification (State ID, Driver’s License, et cetera)
- Medicaid Card or letter from DHS verifying benefits
- List of all medications that you are taking
- List of all doctors that you are seeing with their contact information (Primary care physician, psychiatrist, et cetera)
Income verification (social security/disability award letter, pay stubs, et cetera) - Guardianship paperwork (if applicable)
